A rule-based scheduler can apply logic. It cannot learn whether the logic is still the right one. That is the fundamental difference between a static operating-room system and an RL-powered one.
Why rules break down
Hospitals do not operate in stable conditions. Procedure times drift by surgeon, specialty, weekday, case complexity, anesthesiologist availability, and pre-op readiness. A rules engine usually reflects the assumptions of the week it was configured.
What RL optimizes for
- Higher utilization without pushing avoidable overtime into the evening
- Better sequencing under uncertainty, not just best-case assumptions
- Safer insertion of emergency cases into an already moving schedule
- More accurate prediction of downstream consequences when one case slips
What changes for the hospital
The impact is not abstract. Hospitals recover revenue through tighter block use, fewer dead zones between cases, better first-case preparation, and less uncontrolled cascade delay. OT teams spend less time reacting and more time approving model-supported decisions.
In practice, the strongest benefit is confidence. Once a team sees that the model understands its real operating pattern, it becomes easier to trust a recommendation that would have previously required six phone calls.
